Our Services
The Carers offers a comprehensive range of home care services that is only limited by each person’s available funding whether that is through a home care package (HCP), the Dept. of Veterans Affairs funded services or privately funding services. Our Care Coordinator can advise the best way to support each individual person to remain living as independently as possible in their own home, especially when dealing with complex and changing health conditions.
Care Management
- Assess each client’s individual needs
- Develop a personalised care plan that outlines the client’s services, goals, and outcomes.
- Arranging services and support such as personal care, nursing, allied health, transport, or home modifications.
- Ensuring smooth communication between service providers, GPs, and family
- Regular monitoring and reviewing ensuring the services are effective and aligned with each person’s individual needs.
- Adapting the care plan as the person’s circumstances change.
- Advocating for a client’s best interests and supporting decision-making especially when cognitive decline is present.
Personal Care
- Assistance with showering and bathing
- Dressing and undressing
- Toileting assistance and incontinence management
- Grooming including hair care, oral hygiene and shaving
Food Services
- Preparation of meals
- Heating and plating prepared meals
- Purchasing approved provider meals

Respite

Social Support

- Accompany and participate in community engagement and social activities such as attending café or restaurant for a meal out, attending an art gallery, having fish & chips at the beach etc.
Transport
- Accompany to medical appointments

- Social events and special occasions such as weddings or significant birthday parties.
