Your Home Care Plan

The Carers will work in conjunction with you and your family to deliver a Home Care Plan that is tailored to meet your individual home care needs.

We offer a range of home care support services to assist you to remain living in your own home as independently as possible for as long as possible. Each service type and outcome is documented in detail in your home care plan and can include:

  • Domestic assistance
  • Food services
  • Shopping
  • Personal care
  • Respite
  • Medication management
  • Social support
  • Transport
  • Clinical support

Regular or Casual Home Care Plan

Our services are delivered according to your home care plan and our home care services are available to you on a regular or casual basis, 24 hours per day, seven days a week.

Your home care plan is delivered by our Home Care Support staff who have been security screened, and have the necessary skills and qualifications enabling them to work to the standards set by The Carers while observing the National Home Care, Department of Veteran Affairs and the Home and Community Care (HACC) Service Standards. We provide ongoing support to our home care support staff through our Staff Management program which includes regular training and staff performance appraisals.

In managing your Home Care Plan, The Carers is committed to provide the most suitable staff to meet your specific needs. We understand the importance of ensuring continuity of your home care services and aim to keep any changes to your staff to an absolute minimum.

The Carers are committed to providing personal communication to our clients and their family. We make every effort to have one of our skilled Care Coordinators available to answer your calls at all times, no pressing numbers – real personalised service. We aim to keep you informed of any changes in your home care support plan or staff, your rostered home care service time or dates or any other relevant information. We want to do all we can to ensure you are satisfied with your home care plan and the home care services delivered by The Carers.

When scheduling our Home Care Support Staff our Care Coordinator ensures only staff who have the required skills and experience will be allocated to perform the home care services detailed in your home care plan. We seek personal qualities in our home care support staff such as good communication skills, ability to use their initiative within the boundaries of individual clients home care plan, being professional while maintaining a caring and empathetic approach and having a commitment to the home care philosophy.

As an organisation we are committed to working with you and your family and encourage your participation in the wellness and enablement philosophy when developing your home care plan. We work within this framework to assist you develop, retain and/or regain independence in your physical, social and emotional functioning; allowing you to continue to live independently in your own home. Our Home Care Staff will support and encourage you to focus on your strengths and abilities and will work with you promoting the positive outcomes that can be achieved within your care plan.
